THE RECORD PROCESS

Prepare your information, submit it for review, and keep your application reference. Documents are created after the office has reviewed and approved a record.

Prepare the profile

Check the handler name, dog name and breed. Choose a clear, recent portrait. Optional personal fields can be left blank.

Send the information

Use the information form and save the reference shown on your receipt. Your submission enters the office review queue.

Review and correct

The office reviews the details. Contact us with your reference if a name, photo, or measurement needs correcting.

Keep your documents

Approved records can be used to generate a training certificate and a two-page card PDF. Keep the original file and use the record reference for later questions.

What the record means

Our documents describe records held by an independent provider. They do not create service-animal status or confer rights under the ADA. See our U.S. guidance page for official sources.
